| Grant Report Form
After you have received your grant, and the funded project is completed, you are required to electronically submit to the Brookline Community Foundation a Grant Report Form. This form must be submitted within six weeks of having completed your grant. Please download a grant report form to complete this requirement. When completed, please attatch the form electronically and email it to SDassel@brooklinecommunity.org. |
