1. BCF Grant Application, Part 1 (Excel)

2. BCF Grant Application, Part 2 (Supporting documents)

Please complete Part 1, the Excel spread sheet, and include a narrative and all required documents from Part 2. Documents for Part 2 should be sent as a single PDF file. 

Send the completed application electronically to jlemire@brooklinecommunity.org. Please note the total number of attachments in the body of your email.
